Attached you will find the plan offerings that are available.<\/p>\n<p>We are happy to announce the monthly premiums for the Guardian dental and vision plans are remaining the same and there is no change to the plan coverage. Slight adjustments have been made to some of the other Guardian offerings effective <span data-term=\"goog_1218675989\">7\/1\/2017<\/span>:<\/p>\n<ul>\n<li><strong>Guardian Voluntary Life Insurance for Children:<\/strong> If the employee purchases at least a $20,000 voluntary policy for themselves it allows them to purchase either a $5,000 policy for their child(ren) for $1.00\/month or a $10,000 for their child(ren) for $2.00\/month.<\/li>\n<li><strong>Guardian Voluntary and Employer Sponsored Long Term Disability:<\/strong> \u00a0The calculation has increased from $0.18 per $100 of benefit up to $0.30 per $100 of benefit.<\/li>\n<li><strong>Employer Sponsored Dental Preventive Maximum Waived:<\/strong> Starting July 1<sup>st<\/sup> preventive service claims will no longer go toward the dental annual max for groups offering Employer Sponsored Dental.\u00a0 This potentially could save each member $600 per participant on their plan.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<ul>\n<li><strong>All other added-on benefits provided by Guardian remain the same!<\/strong><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Please let me know if you wish to make any changes to your Guardian plan offerings (i.e. dental, vision, life, disability) or contribution structure for the upcoming benefit year no later than <span data-term=\"goog_1218675991\">April 10, 2017<\/span>. \u00a0\u00a0<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p><strong>Current participants with Guardian will not need to complete any forms unless they are making any changes, newly enrolling, or canceling the plan.\u00a0 All employee changes must be reflected in our system no later than <span data-term=\"goog_1218675992\">May 15<\/span> for the July 1 effective date.<\/strong><\/p>\n<p><strong>\u00a0<\/strong><\/p>\n<p>If you have any questions regarding the above information, please contact me via phone at <a href=\"tel:(248)%20504-6567\">(248) 504-6567<\/a> or via email <a href=\"mailto:kneff@apteam.com\">kneff@apteam.com<\/a>.<\/p>\n<p><em><strong>Please let me know if you would like further information on these benefits, would like to add, change or delete.\u00a0 Everything needs to be submitted by April 10th.\u00a0 This is available for employees who work 30+ hours per week.<\/strong><\/em><\/p>\n<p><em><strong>Please initial once you have read and acknowledged this info.<\/strong><\/em><\/p>\n<p>Thank you,<\/p>\n<p>Erika<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Open Enrollment for AccessPoint\u2019s Guardian ancillary plans is set for July 1, 2017.
